Older Adults & Home Health (OA&HH)

• Older Adults & Home Health provides expertise and specialist care for older people
• Our care is both in an inpatient and community setting
• We recognise the importance of interaction between physical, emotional, social and family for patient wellbeing
• We work with patients and their families/ whanau as partners in care
• We liaise with other hospital services and care providers as necessary
• Community services provide home based care for adults (16 years and over) 
• We care for a range of health needs and disability support requirements, including:
• follow up care after hospitalisation
• palliative care for terminal illness
• conditions which require the care and treatment from a nurse and/or therapist
• Close links are maintained with all other health services particularly:
• Allied Health
• inpatient hospital services
• GPs and other primary care providers

• Assessment Treatment & Rehabilitation Service (AT&R)
• Continence Service
• District Nursing
• Gerontology Nurse Specialist Service (GNS) - Nga Kaitiaki Kaumatua
• Mental Health Services for Older Adults (MHSOA)
• Needs Assessment and Service Co-ordination (NASC)
• Ostomy Service
• Palliative Care
