Waitemata DHB has a 24-hour Clinical Records Service based at North Shore Hospital in Takapuna. There is also a Monday to Friday satellite service based at Waitakere Hospital.

District Health Boards and Hospitals requiring information for ongoing acute care, please send a fax with your request to 09 488 4622.

Please include on the fax your name, your title and your fax number; the patient’s NHI, first name, surname, and date of birth; details of exactly what information you require (e.g. discharge summary from operation; clinic letter from rheumatology, imaging result from angiogram) and for what date or date range. Detailed information will help speed up our response to your request for information.
