This Plan is a higher level summary of action areas of targeted effort to increase health gain where health inequalities impact on particular high risk groups for our Asian, new migrant, former refugee and student populations.

Achieving better health outcomes, not only in hospital but in the community, is the aim of the Primary and Community Service Plan launched by Waitemata DHB in partnership with Waitemata PHO and Procare Health Ltd.
The System Level Measures improvement plan is a joint-plan between Auckland Waitemata Alliance and Counties Manukau Health Leadership Teams. This is a single improvement plan to ensure streamlined activity and reporting and best use of resources within the health system.
The health plans have been developed collaboratively in partnership with both MOU partners and with our PHO partners. Where possible, Māori health gain activities have been aligned across Waitemata and Auckland DHBs. As a result, the two Māori health plans contain identical activities for the majority of priority areas and the local priorities aligned.
